Lauralton Hall Student/Parent Handbook of Standards and Expectations
The Handbook provides information on the school’s standards, policies and procedures.
The following addendum to the Lauralton Hall Student/Parent Handbook of Standards and Expectations (Section VI-B: Violations – Mobile Device Use) was adopted in October 2011:
Teachers may confiscate a mobile device if it is visible in a class. A student must see the Dean of Students to have the device returned. Any written violation related to improper use of a mobile device or any confiscation of a mobile device by a teacher will result in a detention from the Dean of Students.
Emergency Information/Handbook Agreement Form
To ensure that students are able to receive prompt medical attention in case of emergency, Lauralton Hall parents complete, sign and submit the Emergency Information/Handbook Agreement Form on or before the first day of school each year. The form is sent to all parents each summer. In addition, students and their parents are required to read the Student/Parent Handbook and sign in the Student/Parent Handbook Agreement box on this form indicating that the information in the Handbook is understood and agreeing to be governed by it.
Health Assessment Record Form
This form requests information on the student’s health including information on primary immunizations and a health assessment performed by a medical professional during a medical examination.
- Students are required by state mandate to have a medical examination sometime during the sophomore year and to submit the completed Health Assessment Record Form to Lauralton Hall in order to enter junior year. At the end of freshman year, parents of freshmen are reminded to download the form and schedule a physical examination. State law mandates that students must submit a completed Health Assessment Record Form in order to enter grade 11.
- Annual current physical examinations are required of students who participate in varsity, junior varsity or club sports. The Health Assessment Record Form must be completed and returned to school marked for the Athletic Director prior to the first day of team tryouts. A current (within thirteen months of the physical exam) Health Assessment Record Form must be on file at school in order for a student to try out for, practice or play a sport at Lauralton Hall. According to CIAC regulations, if the form expires during the playing season, a new form is required without lapse or the student will not be able to continue participation.
- Incoming students from Connecticut schools must submit a copy of the Health Assessment Form reflecting the most recent physical examination by August 1 of the student’s first year at Lauralton Hall. This can be requested from the health office of the student’s current school.
- Students from out of state must submit a health assessment completed within twelve months prior to the student’s date of entry into Lauralton Hall. The health assessment must include an immunization record and the student must be in compliance with the Connecticut immunization requirement.
Authorization for the Administration of Medicine Form
The Milford Board of Education and the Department of Health, as a service to students, allow the administration of medications in school. By law, the State of Connecticut regulates this service. Medication to be administered or self administered at school requires completion of the
Authorization for the Administration of Medicine Form by the physician and the parent/guardian. If a student is to self administer the medication, she must have written permission from the physician and parent, and she must be assessed by the school nurse as competent to self-administer. All medications to be administered at school are to be delivered to the school nurse by an adult. The medicine container is to be labeled with the student’s name and the directions for administration. The pharmacist may be asked for a separate labeled container for school if needed. This applies to all medications, both prescription and over-the-counter.
Authorization for the Administration of Medicine Forms will be honored for the dates noted on the form, or in the case of daily/emergency medicine, for the current school year.
Health Care Provider Verification of Vaccination/Disease Form
Students entering grade nine and other students new to Lauralton Hall must submit a
Health Care Provider Verification of Vaccination/Disease Form that has been completed and signed by a health care professional by August 1 of the student’s first year at Lauralton Hall.
Milford Health Department High School History Form
Students entering grade nine and other students new to Lauralton Hall must complete and submit a
Milford Health Department High School History Form by August 1 of the student’s first year at Lauralton Hall. This form provides information on the student’s health history.
Student Medical Record Update Form
This form should be used to update a student’s school health record whenever there is a change in health status, a new medical diagnosis, or
any new medication. Parents/guardians should always contact the school nurse immediately with any health issue that directly impacts school attendance or access to school to ensure the student’s safety during the school day and allow for any accommodations the student may need to safely access the school buildings. .
Lauralton Hall Athletics Handbook
The Athletic Handbook presents a wealth of information about participation in sports at Lauralton Hall including the rules and regulations of the Connecticut Interscholastic Athletic Conference (CIAC). The Handbook is given to parents of athletes at the annual mandatory sports parents meeting held each summer and is available in the office of the Athletic Director.
Interscholastic Sports Permission Form
Student athletes and their parents are required to complete, sign and submit this form prior to participating in any varsity, junior varsity or club sport at Lauralton Hall. The form provides a warning statement that participation involves the potential for injury that is inherent in all sports. Parents provide emergency medical information and, by signing the form, acknowledge acceptance of the rules and regulations set in the Lauralton Hall Athletics Handbook.
Senior/Junior Privilege Permission Form
In order for students to be afforded junior/senior privileges, parents of seniors and juniors must complete and sign Senior/Junior Privilege Permission Form.
